Oak Pointe Nursing and Rehabilitation Center, under Foundations Health Solutions, an 89 bed facility located in beautiful scenic Amish country, is currently looking to fulfill positions for STNA’s.

Job Title: State Tested Nurse Aide
Department: Nursing
Reports to: Floor Nurse, Assistant Director of Nursing, and Director of Nursing

General Job Summary:

The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors, while promoting independence in accordance with safety standards. Fosters self-respect and a feeling of worth in each resident by consistent understanding and kindness in care. Observes and reports changes in residents’ physical and emotional condition to nurse or supervisor.

Job Description:

Assists residents with Activities of Daily Living (ADL’s), including bathing, dressing, eating, toileting, and transferring. Properly lifts residents according to needs determination, using proper assistance (i.e. 2 STNAs, Sit/Stand Lift, Hoyer Lift)
Makes resident beds, changes linens, and organizes rooms to maintain a safe environment.
Answers call lights in a timely manner.
Assists new residents in transition to facility – labeling and putting away belongings, advising of meal and activities schedules, etc.
Maintains clean work rooms, records resident data on various logs, and properly stores equipment.
Attends required in-service training and meetings.
Other tasks as assigned within scope of practice.
Maintains Standard Precautions at all time to ensure proper infection control.

  • Must be certified by the State of Ohio or enrolled in our PAID STNA class program.
  • Must obtain certification within 45 days of hire.
  • Must have basic communication skills and a willingness to learn and be taught by others. Must be able to read, write, and follow oral instructions.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of team work and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health and safety of other individuals in the workplace.

Work Experience:

Previous health care-related experience preferable, but not essential. On-the-job training available. Will work under close supervision, but must be self-directed and motivated.

Physical Demands:

Good physical and mental health. Must be able to endure constant standing and walking throughout shift. Must be able to turn, stoop, bend, stretch, and lift to complete above job tasks. Must have good finger and hand dexterity to handle delicate equipment. Must have good visual and hearing acuity to detect changes in residents’ conditions and needs.
